D25 Saline

D25 Saline (25% Dextrose in 0.45% or 0.9% Saline)

• Composition (per 100 mL):

  • Dextrose: 25 g (25%)

  • May be combined with 0.45% or 0.9% sodium chloride depending on formulation

• Osmolarity: Very high (>1250 mOsm/L)

• Uses:

  • Acute hypoglycemia management in ICU/ER

  • Temporary calorie source in parenteral nutrition

  • Neonatal resuscitation (in some protocols)

• Benefits:

  • Rapid glucose restoration in emergencies

  • Concentrated energy support for critical care

• Caution:

  • Must be administered via central line or slow IV

  • Risk of vein irritation, hyperglycemia, fluid shifts, and electrolyte disturbances

  • Not for routine maintenance fluid use
